Managing the Obese Patient
Understand the risk management/patient safety/patient care considerations in the treatment of an obese patient. Presentation content will include the following eight areas: Communication/sensitivity, Environmental concerns, Equipment requirements, Clinical risks, Surgical risks, Risk of misdiagnosis, Treatment strategies including risk factors and Discharge planning and follow up.

Informed Consent
The doctrine of informed consent is explained, including legal liability for failure to obtain, but also the legal protections that accrue when it is properly obtained. Situations in which it must be obtained and the risks that should be discussed are explained. The responsibility for obtaining informed consent is discussed. The nurses role in informed consent is also discussed. CMS requirements for informed consent are also discussed.
This program is appropriate for all care settings and can be customized to any provider/staff mix audience.

Clinical Staff and Physician Interactions
“Communication Between Caregivers – A Corner Stone For Patient Safety”
The communication barriers that occur between clinical staff, nurses and physicians will be discussed.  A scenario-driven situation will be presented  that will provide participants with opportunities to identify several communication related issues including: Bullying, Chain of Command, Addressing patient care issues - patient advocate,  Conflicts and miscommunications, and when to have a crucial conversation.  Tips for improving communication to assist in developing a high-reliability patient care team that speaks the same patient safety language will be shared. This program is appropriate for any setting and can be customized for any provider and/or staff mix audience.

Peer Review
The duty of the health care organization and its medical staff to assure that the clinical performance of all members of the medical staff is subject to internal quality monitoring and peer review is not a new obligation. The intent of quality monitoring and peer review is to promote an effective evaluation of the quality and appropriateness of care and to assist in identifying opportunities to improve patient care on an ongoing basis. However, medical staff quality monitoring and peer review presents special challenges. This program will include discussion of medical staff Peer Review and the role risk management plays in this important responsibility. This presentation will focus on the following: Peer Review Regulatory Standards and Liability, CMS Conditions of Participation for hospitals and CAH, The Joint Commission, Peer Review Protection Laws, and Barriers to Effective Peer Review and Possible Solutions.

Documentation
The medical record is a formal history of the patient’s illness, symptoms, diagnosis, treatment, and response to treatment.  Not only does the medical record document the care given to the patient, it is also the principal communication among physicians and other health care providers about that care.  The presentation covers basic techniques for documenting appropriately, including specific tips for more effective documentation. Common pitfalls and examples of inappropriate documentation are given. This program is applicable to all health care settings, and can be customized to any provider/staff audience mix.

Credentialing/Privileging
This program will define the common key elements of effective credentialing, privileging, reappointment and peer review.  The difference between credentialing and privileging, regulatory requirements for appointment/reappointment and the six general competencies and their application will be discussed.  The six general competencies include: Patient care, Medical/clinical knowledge, Practice-based learning and improvement, Interpersonal and communication skills, Professionalism and Systems-based practice.  Application of the general competencies to Ongoing Professional Practice Evaluation (OPPE) is included.

Key Components of an Effective Infection Control Program
The purpose of this infection control educational program is to provide insight into the key components of an effective infection control program, which includes critical elements impacting infection control in health care settings and reporting requirements associated with infection control activities, evaluation, analysis and findings. A discussion of the required Infection Control Risk Assessments (ICRA) is included.

Physician Office Practice Risk Management
Today's clinical practice places extraordinary demands on physicians and errors can happen to anyone. With the continual threat of legal action, how can physicians, nurses, and support staff deal effectively with these challenges and ensure that patients are safe? This presentation will address some of the top risk management issues faced today including medication reconciliation, informed consent, critical test tracking, disclosure of unanticipated outcomes, and documentation.

The Disruptive Clinician
Misconduct of any kind by any physician or other health professional has no place in the practice of medicine. Disruptive behaviors contribute to a decrease in staff morale and have a negative impact on patient safety.  This program focuses on defining disruptive behavior, discusses the triggers for disruptive behavior, examines the impact of disruptive behavior on clinical, financial, and environmental operations, and finally explores proactive and reactive strategies to address disruptive behaviors. A sample Code of Conduct will be provided.  This program is appropriate for any setting and can be customized for any provider and/or staff mix audience.

EMTALA
Compliance with the ever-changing federal Emergency Medical Treatment and Labor Act (EMTALA) may include fines and penalties - CMS and OIG, significant legal implications, non-coverage by insurance, NPDB reporting and punitive damages.  This  program will address all of the federal definitions associated with EMTALA, including, but not limited to, triage, medical screening examination, ED, stable, transfer and qualified medical personnel.  The process required to comply with the federal EMTALA regulations is discussed.  Presentation content will also include a discussion of the legalities, liability exposures and penalties for non-compliance with EMTALA. This program is focused towards the hospital setting and is applicable for emergency physicians and staff. It can also be customized for on-call specialists. 

Dealing with Dissatisfaction, Complaints, Grievances & Disclosure
Discuss strategies to address patient complaints during the care process and those identified after discharge. The differences between a complaint and a grievance will be shared.  Federal and regulatory guidelines for complaint management are discussed. Scenarios and solutions for resolution of complaints and dissatisfaction are shared.  Tips for an effective disclosure are reviewed.

Risk Management Concerns and Solutions:  COMMUNICATION and COORDINATION of CARE in the Ambulatory Setting:
The purpose of this program is to identify the scope of medical error in the ambulatory setting and steps to prevent these errors in your practice.   Day to day strategies to improve communication with patients and other health care providers will be discussed.  The impact of medical literacy will be shared as well as techniques to address this pervasive issue.  A focus of this program is communication of critical tests and other areas of communication pitfalls in the physician practice setting including informed consent, medication management, telephone advice, ‘no-shoes’/cancellations and termination of care.
 
OB EMTALA Legal Issues
This program will focus on the Emergency Medical Treatment and Labor Act (EMTALA) as it pertains to the OB department.  The history and background of this federal regulation is discussed.  The key elements of EMTALA that have an impact on the OB department include: Triage versus Medical Screening Exam (MSE), Qualified Medical Personnel (QMP),  Emergency Medical Condition (EMC),  Stabilization and Transfers. Documentation requirements for both the sending and receiving facility will be presented.

A Look at the Top Ten Perinatal Risks that Threaten Patient Safety
Every expectant parent has the expectation of a good outcome. Most babies are born health and most mothers experience labor and delivery without significant adverse events. This program will acknowledge and identify the challenges, discuss the risks and  pinpoint the top ten patient safety threats encountered in perinatal units.  The five areas of highest risk for obstetrical harm will be identified. Strategies to assist your organization in taking your perinatal unit to the  next level of patient safety will be discussed. The target audience includes OB Medical and Nursing staff, Residents, and Risk Managers. This presentation is hospital-focused
