Consistent, accurate and timely documentation promotes patient safety through clear communication between disciplines. Special considerations are important to recognize when using various documentation formats such as paper and electronic methods. When legal action occurs, attorneys are adept at teasing out inconsistencies and inaccuracies in the medical record. Therefore, documentation best practices remain an important component in obstetrical care. This program discusses obstetrical documentation best practices and pitfalls, assists the provider in understanding how appropriate documentation mitigates risk of liability, and provides documentation recommendations for high-risk obstetrical presentations.
As a result of this presentation, participants will be able to: • List the six general purposes of medical record documentation • Recognize five documentation errors • Recognize three challenges of using the various medical record formats • Identify three OB documentation best practices for high-risk presentations
