Insufficient hand-offs in clinical practice have been identified as a leading cause for medical errors. Many health care providers struggle with this issue. This important program will provide information on how to identify problems and conduct effective hand-offs in clinical practice. A checklist outlining the necessary components of an effective handoff will be provided.
