Clinical Risk Assessments


 
 
Emergency Medicine, Obstetrics, Behavioral Health, and Surgery continue to be service lines with potential exposure to risk for many organizations. Recognizing the individual needs and financial resources available in different organizations, The Risk Management and Patient Safety Institute (The RM&PSI) provides four levels of services.
 
We are committed to conducting Clinical Risk Assessments that provide you with valuable and actionable information complete with recommendations substantiated by experts in the pertinent clinical field and in risk management, patient safety, and quality.
 


The RM&PSI four levels of assessment services:

 
  • 1-1½ days onsite* review of selected clinical practice (ED, OB, BH, OR, etc)
  • Delivery of care, processes, and outcomes evaluation for selected clinical practice
  • Facility and medical staffs interviews to determine strengths, weaknesses, opportunities, and threats
  • Document review of pertinent policies, procedures, and forms
  • Focused medical record documentation review
  • Evaluation of clinical risks and potential liabilities related to selected clinical practice
  • Onsite closing conference with stakeholders to discuss findings
  • Full report of findings, recommendations, and supportive rationale complete with references
  • A risk management consultant to coordinate the assessment

 

 

 2 Star Assessments include 

  • Includes all 1 Star Assessment components
  • A clinical expert** in addition to the risk management consultant for greater depth and scope of review
  • An enhanced focused medical record documentationreview
  • Expert observation of clinical practice and processes
  • Enhanced report related to clinical expert input

 

 

 3 Star Assessments include:

  • Includes all 2 Star Assessment components
  • Multiple days onsite* for comprehensive review of selected clinical practice (ED, OB, BH, OR, etc)
  • Evaluation of organizational approach to patient safety
  • Facility and medical staffs survey to assess predominant culture and attitudes related to patient safety
  • Comprehensive document review (offsite-policies and procedures; onsite-confidential documents)
  • Comprehensive review of medical record documentation related to selected clinical evaluation

 

 

 4 Star Assessments include:

  • Includes all 3 Star Assessment components
  • A team of clinical experts coordinated by a managing risk management consultant
  • Comprehensive interdisciplinary observations of practices and processes
  • Multidisciplinary expert medical record documentation review related to selected clinical evaluation
  • Enhanced report related to multidisciplinary input

 

 

* Time on-site dependant on size and complexity of organization.

†  Reports are delivered electronically and include action plan and executive summary.

**Clinical experts may be a physician, registered nurse, or other appropriately credentialed clinician (or combination of these) dependant on clinical area assessed and level of assessment.
